I remember finding the NPC invader "Paladin Leeroy" in Dark Souls and being dumbfounded that a game that serious would pay homage to a silly meme that was already six years old.

At least it beat Duke Nukem Forever to the punch.

By the time Duke Nukem Forever finally came out, there was little chance of it beating anyone else to the punch on anything.

The first time I saw it "escape containment" was Bob the Blob in Monsters vs. Aliens. (I no longer remember if it made it to the screen, or was only in a DVD deleted scene extra, though.)
